ABSTRACT
Se presenta la experiencia de 18 pacientes sometidas a cirugía laparoscópica por DIU intraabdominal, en las cuales el diagnóstico se realizó por ecotomografía transvaginal y radiografía de pelvis. En todos los casos su extracción se realizó sin complicaciones ni conversión a laparotomía. Se discute el cuadro clínico, diagnóstico, tratamiento y eventuales complicaciones.

To define the role of computed tomography [CT] in localization of the perforated uterine devices [UDs]. The study included 30 patients referred to the diagnostic imaging department for localization of missed UDs that were proved to be present in the body by plain X-ray, Ultrasonography showed improper location of UDs. Each patient was subjected to non-contrast axial CT examination according to the site of the device that was seen in the topogram of abdomen and pelvis. CT determined the site of perforated UDs with sensitivity 100%. There were five devices partially perforating the uterus. Three of them were partly in the uterine cavity, myometrium and peritoneal cavity. Two of them were partly in myometrium and peritoneal cavity. One patient had two perforating devices at the same time rendering the number of missed devices 31. The remaining 26 devices were completely perforating the uterus with only one of them perforated the colon. CT localization of perforated contraceptive devices is mandatory before surgical extraction. It can determine the type of perforation whether partial or complete. It can detect associated adhesions or visceral involvement

Se describe un caso en una mujer postmenopáusica con piometra, cuyas manifestaciones clínicas correspondieron aun cuadro de peritonitis generalizada perforado. En la literatura inglesa se informan 15 casos y únicamente dos debido a cáncer cervical, siendo éste el tercero informado con detalle. Se presentaron cuatro casos clínicos de perforación uterina parcial por DIU (T de cobre) en los cuales el diagnóstico se realizó con sonografía. Las principales manifestaciones clínicas encontradas fueron, la dificultad al intentar la extracción del DIU, con o sin sección de los vástagos, y la desaparición de los vástagos en el control rutinario. En todos los casos existía el antecedente de inserción puerperal del DIU durante el período de lactancia. Los signos sonográficos que permitieron realizar el diagnóstico fueron la localización excéntrica y/o la observación parcial del DIU fuera de la cavidad uterina. El DIU fue extraído vía transabdominal en dos de los cuatro casos presentados ante reiterados intentos fracasados de extracción transcervical. Se discuten las principales medidas que permiten aumentar el rendimiento de la sonografía en la detección de esta complicación y su manejo
